It was NOT a perfect fit, it didn’t sit smoothly on the tank. But I have several of the same model toilet and moved it to another one where it sat a bit better.
No - this doesn’t actually fit all the models noted. Had to go to Kohler who could actually match to my toilet

It did not fit properly on my new Santa Rosa toilet. I was trying to replace the one that came with it, because it did not fit properly either. I returned the top.
It fits and the color is exact.
We broke the original cover, ordered the required part nbr via Amazon, received the wrong part. Then we ordered the required part nbr from Kohler and received the correct item. Advice: order directly from the manufacturer when possible.
