For the cable side I simply threaded the connector through...
For the cable side I simply threaded the connector through the front of the plate and onto the end of the cable; no need for the screw holes above and below the cable hole. I'm not sure how they'd be used, and they're still just open (I may glue a screw into each to make it appear complete, but it's mostly hidden behind the TV). Other than that, the plate works perfectly.
