Yes, it's an original mold--of Dionysus, god of wine. Note the grape leaves and grapes in his hair, plus the fact that it's clearly a dude and he looks slightly drunk/hung over. If it were Athena she'd be wearing a helmet which she was wearing when she popped out of Zeus's forehead.

How to apply to door would have been helpful to include as instructions and there was no hardware, such as nails or screws , that came with it.

I ordered a pair of these for my double entry doors. They are beautiful and heavy. You have to purchase your own screws to mount them, but that was not a problem at all. Shipping was fast and the item was as described, so I'm very happy with my purchase.
